Diabetes Self-Management Education and Support Program
Diabetes self-management education and support is an evidence-based program designed to engage people with diabetes in setting goals, developing self-care skills, and adopting healthy behaviors in order to improve their quality of life and reduce their risk of health complications. This program is available to all Minnesota state employees with a diabetes diagnosis who participate in the State Employee Group Insurance Program. Minnesota Department of Health contracted with Wilder Research to conduct a literature review and focus groups to inform communication strategies that encourage program participation.
