Community Metrics: 2016 Summary Statistics, January Through June

The East Metro Mental Health Roundtable aims to accelerate improvements in the Twin Cities east metro mental health system through partnerships that deliver high quality mental health services. They developed community metrics to track progress toward their goals of reducing barriers to patient flow between hospitals and community services, better matching available resources to needs, and reducing gaps in the continuum of care.
